Your First Visit
Your first visit to our office is a great time to get to know our surgeons and staff. We will walk you through your oral health goals and create a treatment plan that’s right for you.
At Balaci Oral and Facial Surgery, we want you to feel welcomed and at home in our office. We are happy to offer several amenities to help you feel more comfortable.

What to Bring
New patients can complete and submit registration paperwork online ahead of the consultation. We encourage you to do so. Otherwise, please arrive at your appointment a few minutes early to complete the necessary paperwork. Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip
- Completed medical or dental insurance forms, if applicable; this will save time and allow us to help you process any claims
- X-rays; if your dentist or physician has taken X-rays, you may request that they forward them to our office or pick them up and bring them to our office yourself.
- A list of current medications
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
What to Expect
At your first visit, you will meet with Dr. Balaci, Dr. Forbes, or Dr. Murphy. We will evaluate your current oral health and talk about any issues you are experiencing. Then, your oral surgeon will create a custom treatment plan unique to your needs. It’s a good idea to come prepared with a list of questions so that we can address all of your concerns.
Our office is equipped with state-of-the-art 3D CBCT imaging technology to assist in accurate diagnostics, treatment planning, and predictable clinical outcomes.
Financial & Insurance Information
We will send you a monthly statement following treatment. Please remember that you are responsible for all fees charged by our office, regardless of your insurance coverage. If you ever have questions about your account or would like to learn about ways to afford your oral surgery care, please contact our office. A member of our administrative staff will be happy to assist you. We accept the following payment options:
